2025-2026 Sports and Activities

Please submit ALL the completed paperwork prior to the first day of practice.  All participating students must turn in a current health exam on file. When registering to participate, please complete the following forms:

• Health Exam form signed by a doctor, student, and parent/guardian. Health exams are good for 18 months and must be current throughout the season of the sport.
• Middle School Activity Participation Form - form completed by parent and student to give consent to participate in activities.‌  Parent and student email addresses must be different as the signatures must be completed separately. 
• Paperwork should be turned in to our AAFP in the office.
• Athletic shirts will need to be purchased for the following sports: XC Running, Girls Basketball, Boys Basketball, Volleyball and Track & Field.  Athletic shirts ($25) can be purchased through Parent Connect or cash/check (payable to Goldenview) and picked up in the office. If paying through Parent Connect, please send your student with a screenshot or copy of your receipt to pick up a shirt in the office.

Sports run from 4:15-5:45 p.m. Pick up is 5:45 p.m. All students MUST be picked up by 6:00 p.m.
